Authority
We ask parents to keep us informed of their mutual agreements: we want to know who has parental authority, one or both of you.
We also want to know what you have mutually agreed upon when your child undergoes treatment or is referred.
Guardianship
If custody is not shared, we request written confirmation. If there is a family guardian, it is necessary that we receive the contactdetails of the guardian.
We do not report when a visit has taken place to the GP. If you have parental authority, you are allowed to make inquiries. We will provide information depending on the age of your child.
